المقدمة


Doctors in the Sudan work in rural hospitals to:
a- Satisfy a community health need
b- Satisfy a ministry of Health requirement
c- Satisfy a personal wish or desires

The majority of the population in the Sudan lives in rural areas and they are now to some extent adequately covered by health services. The Sudan, being a developing country, is facing shortage of trained personnel, inadequate health care facilities and lack of equipments. These shortages are even more evident and acute in rural hospital are challenging.
These include:
a- Health and medical activities (Promotion of health, prevention and treatment of health and health related problems)
b- Administration of the rural hospital and other healh facilities in the area.
c- Participation in community activities.
d- Supervision activities including the health information system, the referral system, drug revolving fund, economic treatment, control of the endemic diseases and epidemics, and all PHC activities in an integrating manner.
e- Participation in training of paramedical and auxiliary health personnel

This course is offered to expose the students to the real environment of a rural hospital and rural community where a community and a constraint oriented approach is the most appropriate way of delivering health services.

• Geography of the area
• Historical and political background
• Economic profile (Traditional industry, agriculture occupations)
• Social profile (Family composition, customs, habits, etc)
• Health profile (Health & health related problems & services)- environmental sanitation
• K.A.P. study in community about a selected topic.

a critical report about his/her findings and state his/her suggestions and recommendations
a separate section in the report summarizing all activities that he/she has done during the training in the rural hospital.

1 GENERAL OBJECTIVES:

The student is expected to observe, see, help or do what the rural hospital doctor and every one of the health team members is doing or ought to be doing. These are including :
1.1 Curative, preventive and promotive activities
1.2 Managerial activities including the supervisory visits
1.3 Participation in community activities which include: health campaigns, health education and volunteers activities.

2 SPECIFIC OBJECTIVES:

By the end of the rural residency course the student is expected to:
2.1 Know the organization of the health services in the rural community visited
2.2 Draw the organization chart of:
1 Rural Hospital
2 Health centre
3 Dressing station
4 Community Health care Unit
5 The health office in the area (environment health)

2.3 Practical, help or observe the activities performed by the doctor in the rural area.
1 Medical/health activities
2 Administrative responsibilities
3 Supervisory role for other health personnel and health facilities in the area
4 Community involvement

2.4 Do the Following Procedures:
2.4.1 LABORATORY
1 test urine for:
a. Sugar
b. Acetone
c. Bile pigments

1 Parentral therapy (1.M., 1.V.) in children and adults
2 Take blood samples from children and adults
3 Do venisection in children and adults
4 Insert nasogastric tube in children and adults
5 Insert rectal tube in children and adults
6 So P.R. examination in adults
7 Do P.V. examination
8 Perform obstetrical examination
9 Incise abscess
10 Dress wound
11 Suture wound
12 Immobilize fractures
13 Apply and remove plasters
14 Fill in birth certificates
15 Fill in death certificates
16 Work in hospital outpatient with paramedical
17 See or do L.P.
18 See or do Bone marrow puncture
19 See or do Gland puncture
20 See or do Catheterization
21 See o do Super pubic puncture
22 Attend operations
23 Attend post mortems
24 Prepare a monthly statistical report
25 Fill in form(8) under supervision

2.4.3 OFF HOSPITAL ACTIVITIES

1 Visit Health Care facilities and participate in the activities under supervision and/o independently

c. Dressing station
d. Community Health Care Unit
2 Visit Health Office and understand its
functons (environmental)

3 Attend medicolegal (court evidence) with the doctor

2.4.4 COMMUNITY ACTIVITIES

1 Collect statistical data about the rural community
2 Population (Number, age, sex distrbution etc.)

2.5 School health programs
2.6 Present his/her final report to the class.
7.2.7 The student should write a report including the following;
2.7.1 Geography of the area
2.7.2 Historical and political background
2.7.3 Economic profile (Traditional industry, agriculture occupations)
2.7.4 Social profile (Family composition, customs, habits, etc)
2.7.5 Health profile (Health & health related problems & services)- environmental sanitation

2.8 Write a critical report about his/her findings and state his/her suggestions and recommendations
2.9 Write a separate section in the report summarizing all activities that he/she has done during the training in the rural hospital.
